U Design Party Waiver Form

Please submit your child's party waiver here.

Up to two children (per family) can be entered on this waiver. If more children need to be entered please submit a second party waiver.

Permission and Release Agreement

I hereby authorize/consent myself and/or my child to participate in the above-named U Design activities. I understand and agree that in participating in any art class, there is a possibility of physical injury. I agree to release and hold harmless U Design, including its teachers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold U Design liable for any personal injury or any personal property damage that may occur on the premises before, during or after classes.

If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights. 

I also authorize/consent U Design to use any photos, images or likenesses taken of myself and/or my child while at a U Design Studio or at a U Design class for presentation under any legal condition, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.

I understand that there shall be no payment for this release. I understand that I may revoke this authorization at any time by notifying U Design in writing.
